Home Medical Services at Gunnison Valley Health

Home medical services is an important part of Gunnison Valley Health’s continuum of care. Our clinicians use professional standards of care in assessment, diagnostics, outcome identification, planning, implementation and evaluation. Our focus is on treatment, prevention and education.

We not only serve the patients directly but we also serve their families. We work closely with our patients and their caregivers to assist in identifying basic or long-term needs and achieving well established goals.

Our team supports patients as they transition to outpatient services in the community. Through these services, our patients are able to remain in their home and utilize community-based resources.

Who Can Receive Home Medical Services

Home medical services are available to individuals at all stages of life, our patients include:

  • Infants born prematurely
  • Children with developmental delays
  • Children in need of occupational or physical therapy
  • Adults recovery from orthopedic surgery
  • Adults in need of wound care
  • Seniors in need of occupational or physical therapy

Home Health Professionals

Gunnison Valley Health's team of Home Health Professionals include:

Skilled Nurses

Our skilled nurses provide assessment and teaching, medication and pain management, IV therapy, wound care and catheter/ostomy management and many other skilled services.

Physical Therapy

Our therapists provide customized programs that include exercise, manual therapy, modalities and neuromuscular re-education to improve strength, coordination and balance. The therapist will also assist with pain control and promote safe ambulation in home.

Occupational Therapy

Occupational Therapy focuses on independent daily living skills through environmental adaptation, education and exercise programs. Our therapists will customized treatment programs to improve activities of daily living and recommend adaptive equipment modifications to promote home safety.

Medical Social Services

Our Medical Social Services team provides assistance to the patient and family in planning for long-term care, seeking financial resources, assessing community resources and adjusting to changes in lifestyle caused by an illness/injury.
