Complete Pre-Surgical Assessment with One Medical Passport

Thank you for choosing Gunnison Valley Health. In order to provide you with the best care we ask that you register with One Medical Passport and complete an online health assessment. To help our providers and surgical team please complete the assessment promptly so if any further testing is needed it will not delay your surgery date.

Once your One Medical Passport is complete it is sent securely to our surgical team for review. Once reviewed a short text message will be sent to notify you that you are cleared for surgery and you will receive an email with additional pre-operative details. If your One Medical Passport is reviewed and further testing is needed a provider will contact you via telephone. The business day prior to your surgery a surgical nurse will call you with your arrival time to the hospital, review your pre-operative instructions and answer any questions you may have.

Once your surgeon's office has confrmed your surgery you will be sent a link to create your One Medical Passport account via text message. Please follow the link to create your account and complete your health assessment.
