Request Medical Records
Paper Copies:
To obtain a copy of your Gunnison Valley Hospital medical records, please
download the Release of Information form. Please complete all sections
of the form.
Completed Release of Information forms may be faxed, mailed, or brought
into the hospital’s Health Information Management Department at:

Please specify if you would like to pick up the medical records in person,
have them faxed to you, or mailed to you. We are not able to email medical records.
Requests for medical records are processed in the order that they are
received. Medical records will be delivered within 10 to 14 days upon
receipt of the request.

Patient Portal
Gunnison Valley Hospital offers patients the ability to retrieve some of
their medical records via a Patient Portal which can be accessed
here.
Gunnison Valley Hospital provides patients access to specific health information
via an on-line medical record.
If you are registered as a patient at Gunnison Valley Hospital, you will
be able to view the following information on-line upon discharge:
- Summary of Care Record
- Problem List
- Active/Current Medication List
- Active/Current Medication Allergy List
- Allergy List
- Care Plan
A limited number of same-day test and procedure results will also be available
on-line. Information from visits to physician offices are not available
here. Please contact the physicians involved in your care for medical records.
Registering Your Account
These instructions will guide you through the one-time registration process.
It is important to follow these instructions in the order they are given:
- Step 1: Confirmation Email
Check your email inbox or spam folder for the confirmation message to begin
your registration process. The email will be sent from “donotreply@gvh-colorado.org”
and the subject line will state “Gunnison Valley Hospital - Patient
Portal New User.” Open the email and click the link provided. You
will be transferred to a webpage where you will be asked to register as
a new user.
You will be asked to enter your name, date of birth, unique user ID and password.
Make sure you remember/retain your user ID and password for future use.
Very important: Make sure you check the box at the right to verify that
your information is correct. Then click the “register” button
at the bottom of the screen.
- Step 3: Security Question
Choose three security questions, fill in the answers, and then click the
“submit” button at the bottom of the screen. On the next page,
click the on “Main Menu” to begin looking at your medical record.
From the patient list, click on the name of the patient you are authorized
to view. The patient’s visits will be listed chronologically. Next
to each visit there is a link that shows the account number (A) and the
title “Summary of Care” (B). Click on the link in the Summary
of Care column to view the document.

initial registration email sent to you again.

Please call and schedule an appointment with the physicians involved in
your care if you have any questions about your medical records content
or results.
