Step 1 (VISIT): You will need to be seen by a provider to receive an order for monoclonal
antibody treatment. You can:
- Schedule a visit with your primary care provider.
- Go to the Gunnison Valley Health Urgent Care Clinic located at 707 N. Iowa Street in Gunnison.
- Go to the Gunnison Valley Health Mountain Clinic located at 12 Snowmass Road, Axtel 100 in Mt. Crested Butte.
- If you do not have a primary care provider, contact the Gunnison Valley Health Family Medicine Clinic at 970-642-8413 to schedule a visit.
- Step 2 (SCREEN & ORDER): Your provider will complete the required eligibility screening and supply you with the necessary treatment order for Gunnison Valley Health to be your infusion location.
- Step 3 (SCHEDULE): Once your eligibility screening is received by our pharmacy, a representative from Gunnison Valley Health will contact you to schedule a date and time for your treatment.
Please note:
- This is a clinical trial that is operating under an Emergency Use Authorization (EUA). It is not a cure for COVID. It may help decrease the severity of the disease and potentially improve your recovery.
- If your doctor indicates you are a candidate, you will need to bring a prescription from your provider to your appointment.
