Gunnison Valley Health is happy to provide an estimate for any service or procedure, and all estimates are confidential. Because your cost depends on your plan and the specific care your provider orders, the most accurate estimate comes from talking with our team.
Information to have ready
To prepare the most accurate estimate, please gather the following from your provider’s office before you call:
- Is the service scheduled as outpatient or inpatient?
- Which Gunnison Valley Health location or department?
- The name of the provider performing or ordering the service
- The primary ICD-10 (diagnosis) code for the scheduled service
- The CPT/HCPCS (procedure) code(s) for the scheduled service
- Your insurance information — the payer/plan name and your member ID
- Whether the service is related to an accident, workers’ compensation, or another third party
- Any add-ons your provider mentioned — for example, anesthesia, a biopsy or pathology, imaging, lab work, or an implant or device

Understanding your estimate
An estimate reflects the hospital’s expected charges for the service requested. Depending on your care, you may also receive a separate charge from the provider who performs your procedure or reads your results, and some procedures include additional services — such as anesthesia, pathology, radiology interpretation, laboratory tests, or medications and devices — that may be billed separately. Your final cost may differ from your estimate based on the physician’s orders and your individual care. If you have insurance, your plan’s benefits will determine your final out-of-pocket responsibility.
