Late Policy
If you arrive past your scheduled appointment time, you may need to be rescheduled. Please arrive 10 minutes early to your appointment to complete paperwork, pay copays, and any other paperwork necessary.

If you would like to release and/or obtain patient information, click the link below and complete the medical records request form. You can either complete it online or print a hard copy and bring it to the clinic or fax a completed hard copy to 970-641-9017.
