To refer a patient to Gunnison Valley Health, please complete this referral form and submit its contents. We will no longer accept orders via fax.

Referring providers must obtain pre-authorization for insurance coverage for infusion requested.

Once information is received and insurance authorization is noted, we will contact the patient to schedule the infusion.
